Additional lines will be charged 10¢ per minute for domestic calling. If Unlimited minutes are used for non-voice calls, Frontier may convert service to a plan that charges for all long distance calls or where systems allow, charge a 10¢ per minute rate for non-voice long distance calls. Customers with usage inconsistent with normal residential voice applications and usage patterns and Arizona customers who exceed 3000 minutes per month may be required to either select another Frontier long distance plan that charges for all long distance calls, or to cancel service. Security software must be downloaded using Frontier High-Speed Internet service and requires Windows 2000 Professional (SP4 or later), XP Home/Professional 32-bit (SP1 or later), or Vista 32-bit. Maximum Internet speeds may vary.
Frontier Payment Protection (FPP) applies during the life of the initial PPP and will provide a credit in the amount of the monthly basic local phone service charge for one line, not including taxes and surcharges, for 3 months and waive any early termination fee. FPP requires that the customer has no payment arrearages and must have made a minimum of 3 monthly payments and submit proof of job loss.
1. Enrollment
(a) To be eligible for benefits from the Program, (1) you or an authorized user named on your Frontier account (such as your spouse) must be employed in a capacity that qualifies you or the authorized user to apply for state unemployment insurance benefits as determined by the laws of your state of residence, and (2) you must be an active customer with a minimum of Digital Phone Unlimited Nationwide or Statewide long distance voice-calling and Frontier High-Speed Internet (HSI) Max Service with a current Price Protection Plan that includes an Early Termination Fee.
(b) If you activate Digital Phone Unlimited with HSI Max service on or after August 1, 2009, Frontier will automatically enroll you in the Program during the period of your Price Protection Plan. This offer is not available for existing customers who activated prior to August 1, 2009.
2. Making a Claim
You will qualify to receive 3 months of basic local phone service (not including long distance) and have your Early Termination Fee waived if: (1) you or a named authorized user (such as your spouse) becomes unemployed and is eligible to receive state unemployment insurance benefits, (2) you have made at least three monthly payments, (3) your Frontier account is current, and (4) you make a claim within twelve months of activating the qualifying Frontier services. To make a claim, please fax proof that you or a named authorized user is eligible to receive or is receiving state unemployment insurance benefits to 1-386-736-2632. We will also accept a severance agreement from the employer as proof. Please include your name, address and telephone number with the fax. The name on your Frontier account or authorized user must match the name on the unemployment insurance benefits or employer severance documents. Once we receive this information, we will contact you to complete your request.
Peace of Mind Hard Drive Backup offer requires customers to subscribe to the new package described in the first paragraph above with a one-year PPP and a separate $50 early termination fee.
New Frontier customers are subject to credit approval. Frontier reserves the right to withdraw all or any portion of this promotional offer prior to expiration date.
