Lifeline Program

This government benefit program provides a monthly discount on qualifying broadband or phone service to low-income residential customers who meet certain requirements in every state and Tribal land. Frontier is authorized to provide programs such as Lifeline to approved applicants.

Find out if you qualify. Visit the Lifeline Program page on frontier.com. Select your home state from the menu to read the eligibility requirements.

Only one Lifeline discount is allowed per household. That means a husband and wife living in the same household may not each receive the Lifeline discount. Also, a household may not receive the Lifeline discount on both a landline and a mobile phone.

In most cases, you can download the application from your state's page along with a worksheet to help determine your eligibility. (If you can't print your application, contact Frontier Customer Service at 1.800.921.8101 and ask for one to be mailed to you.) In some states you'll get instructions for how to get an application.

Be sure to fill out your Lifeline application completely.

There are 3 basic steps to filling out your application:

Step 1. Enter your personal information

1 Clearly print your name as it appears on your Frontier bill.
2 Enter the complete address where you receive your phone service. This cannot be a P.O. Box.
3 Enter your billing address (the exact address to which your bill is mailed), but only if it is different than your service address.
4 Check the box to show if you're receiving your phone service at a permanent, temporary, or Tribal address. Then fill in your telephone number and date of birth.
5 Enter only last 4 digits of your Social Security Number OR your Tribal Identification Number if you are an eligible resident of Tribal lands. Provide an alternate phone number in case we cannot reach you on your primary phone number.

Step 2. Check your eligibility

Complete part 1 or part 2 of eligibility section—not both.

Part 1 This is for households already participating in one of the government assistance programs listed. Proof of participation is needed. (Send photocopies only; originals will not be returned.)
Part 2 This is for households not participating in one of the government assistance programs listed, but who qualify based on their level of income. Proof of income is needed. (Send photocopies only; originals will not be returned.) Fill in number of people living in your household.

Step 3. Sign and submit your completed application

1 Initial each line in the list of statements. This is very important.
2 Sign your full name on the Applicant Signature line and fill in today's date.
3 If your application is filled out by your legal representative, they must sign and date form and include their phone number.
4 See the bottom of the application for the address where you must mail your completed application.
