Lifeline Program

Frontier is committed to helping qualified individuals, schools, libraries, community based organizations, and health care providers meet their needs. Frontier has been authorized to provide programs such as Lifeline to approved applicants. The discounts are intended to ensure that qualified entities have access to affordable telecommunications and information services.

Lifeline is a government benefit program that provides a monthly discount on home or mobile phone service to qualifying low-income residential customers in every state and Tribal land. To find out if you qualify, visit the Lifeline Program page on and select your home state from the menu to read the eligibility requirements.

Only one Lifeline discount is allowed per household. That means a husband and wife living in the same household may not each receive Lifeline-discounted phone service, and the household may not receive a Lifeline discount on both a landline and a mobile phone.

In most cases you can download the application from your state’s page (in some states you will be given instructions for how to get an application), as well as a worksheet to help determine your eligibility. If you cannot print your application, please contact Frontier Customer Service at 1.800.921.8101 and ask for one to be mailed to you.

Be sure to fill out your Lifeline application completely.

There are 3 basic steps to filling out your application:

1 Clearly print your name as it appears on your Frontier bill.
2 Enter complete address where you receive your phone service. This cannot be a P.O. Box.
3 Enter billing address (exact address to which your bill is mailed), but only if it is different than your service address.
4 Check box to indicate if you are receiving your phone service at a permanent, temporary, or multi-household address, fill in number of people living in your household, your telephone number, and your date of birth.
5 Provide only last 4 digits of your Social Security Number OR your Tribal Identification Number if you are an eligible resident of Tribal lands. Provide an alternate phone number in case we cannot reach you on your primary phone number.
Complete part 1 or part 2 of eligibility section—not both.

Part 1 For households already participating in one of the government assistance programs listed. Proof of participation is needed. (Send photocopies only; originals will not be returned.)
Part 2 For households not participating in one of the government assistance programs listed, but who qualify based on their level of income. Proof of income is needed. (Send photocopies only; originals will not be returned.)
1 You must initial each line in the list of statements. This is absolutely critical.
2 Sign your full name on Applicant Signature line and fill in today's date.
3 If application is filled out by your legal representative, that person must sign and date form and provide their phone number.
4 See bottom of application for address to which you must mail completed application.
