Customers with medical or health conditions that require access to Frontier’s service and warrant special treatment may enroll in Frontier’s priority medical emergency status. Customers with medical emergency status verified by their medical provider are eligible for priority treatment with respect to Frontier service repairs and outages. The Customer’s medical provider may be a licensed physician, physician’s assistant, nurse, or public health official.
Customers may enroll by submitting the Medical Emergency Priority Status Account Enrollment Form. This enrollment would need to include the Frontier form, with your account and physician information, certifying the medical need for priority access to Frontier’s service.
