To validate your identity and complete your service order, please complete the Frontier Positive Identification Form and return it to us as soon as possible. Your service order is currently in a “hold” status until we receive a completed Positive Identification Form.
One form of Non-Expired Government Issued Photo ID is acceptable. Otherwise, two separate forms of Positive Identification are required. The first form must be a valid picture ID. The second form must be a valid state/government issued ID:
